Requesting your Health Records

You have the right to access your health care records maintained by Texas Mental Health Online.

To request a copy of your records:

1. Submit a written request by:

• Email: texasmentalhealthonline@gmail.com
• Secure client portal via Simple Practice
• In person at our office during business hours

2. Include:

• Your full name and date of birth
• The specific records requested
• Your preferred delivery method (secure email, portal, mail, or pickup)

Requests are processed in accordance with Texas law and applicable privacy regulations. If you have questions about accessing your records, please contact us by phone at (940) 432-8631 or by email at texasmentalhealthonline@gmail.com 

If you believe this business has violated consumer protection laws, you may contact the Texas Attorney General’s Office to file a consumer complaint. You can file a complaint or submit a tip online by clicking here.

If you believe a clinician has violated professional ethical standards or acted outside the scope of their licensure, you may file a complaint with the appropriate licensing authority. Information regarding the Texas Administrative Code and the complaint process is available through the Texas Behavioral Health Executive Council.
